تاثیر تمرین همزمان (کانکارنت) بر میزان لپتین، آدیپونکتین و ترکیب بدن در بزرگسالان دارای اضافه وزن و چاق: یک مرورنظام‌مند و فراتحلیل

چکیده

هدف: لپتین و آدیپونکتین دو نوع آدیپوکاین (یا آدیپوسایتوکاین) هستند که در کنترل فرآیندهای متابولیک به ویژه در چاقی نقش مهمی دارند. هدف از مطالعه حاضر بررسی تاثیر تمرین همزمان بر سطوح لپتین، آدیپونکتین و ترکیب بدن در بزرگسالان چاق و دارای اضافه وزن بود. روش شناسی: جستجوی سیستماتیک در پایگاه­های اطلاعاتی PubMed، Web of Science، SID، Magiran و Google scholar برای مقالات انگلیسی و فارسی منتشر شده تا شهریور ماه سال 1402 انجام شد. فراتحلیل حاضر برای بررسی تاثیر تمرین همزمان بر سطوح لپتین، آدیپونکتین و ترکیب بدن در بزرگسالان چاق و دارای اضافه وزن به انجام رسید. اندازه اثر و فاصله اطمینان 95 درصد (CI) با استفاده از مدل اثر تصادفی محاسبه شد. نتایج: نتایج 26 مطالعه با 732 بزرگسال مبتلا به اضافه وزن و چاقی نشان داد که تمرین همزمان سبب کاهش معنادار لپتین سرمی ]001/0 p=،61/0-=[SMD، وزن بدن ]001/0p= ،kg 38/3-=[WMD، شاخص توده بدنی ]001/0p= ،kg/m2 74/0-=[WMD، توده چربی بدن ]001/0p= ،g 56/2-=[WMD، دور کمر ]001/0p= ،cm 47/5-=[WMD، نسبت دور کمر به دور لگن ]001/0p= ، 02/0-= [WMDو افزایش معنادار آدیپونکتین سرمی ]001/0p= ،56/0= [SMDنسبت به گروه کنترل در بزرگسالان چاق و دارای اضافه وزن شد. نتیجه­گیری: تمرین همزمان و ترکیبی سبب کاهش معنادار لپتین سرمی و افزایش معنادار آدیپونکتین سرمی در بزرگسالان دارای اضافه‌ وزن و چاق می‌شود. این موضوع نشان دهندة مزایای سلامت قابل توجه تمرین همزمان برای بزرگسالان است. لذا پیشنهاد می‌شود که جامعه آماری مزبور برای دستیابی به تندرستی از مزایای تمرین همزمان بهره بجویند.

The effect of concurrent training on the levels of leptin, adiponectin, and body composition in adults with overweight and obesity: a systematic review and meta-analysis

Aim:      Leptin and adiponectin are two types of adipokines (or adipocytokines) that play an important role in controlling metabolic processes, especially in obesity. The aim of this study was to investigate the effect of simultaneous exercise on leptin, adiponectin and body composition levels in adults with overweight and obesity. Materials and methods: A systematic search was conducted in PubMed, Web of Science, SID, Magiran, and Google Scholar for English and Persian articles published until September 2023. The present meta-analysis was conducted to investigate the effect of concurrent exercise on leptin, adiponectin and body composition levels in adults with overweight and obesity. Effect size and 95% confidence interval (CI) were calculated using random effect model. Results: The results of 26 studies with 732 overweight and obese adults showed that simultaneous exercise caused a significant decrease in serum leptin [SMD=-0.61, p=0.001], body weight [WMD=-3.38 kg, p=0.001], BMI [WMD=-0.74 kg.m2, p=0.001], body fat mass [WMD=-2.56 g, p=0.001], waist circumference [WMD=-5.47 cm, p=0.001], waist circumference to hip circumference ratio [WMD=-0.02, p=0.001], and increased serum adiponectin was significant [SMD=0.56, p=0.001] compared to the control group in adults with overweight and obesity. Conclusion: Concurrent and combined training causes a significant decrease in serum leptin and a significant increase in serum adiponectin in adults with overweight and obesity. This shows the significant health benefits of concurrent exercise for adults. Therefore, it is suggested that the aforementioned statistical community should take advantage of the benefits of concurrent exercise to achieve health.
