تاثیر تمرین تناوبی با شدت بالا بر مارکرهای التهابی در بیماران دیابتی نوع2 : مرور نظام مند با فراتحلیل

چکیده

هدف: دیابت نوع 2 با افزایش التهاب سیستمیک مرتبط است که ممکن است باعث افزایش بیماری های قلبی عروقی شود که تمرین ورزشی یک درمان بالقوه برای آنها به شمار می رود. با این حال، اثرات تمرینات تناوبی با شدت بالا  (HIIT)بر نشانگرهای التهابی در بیماران مبتلا به دیابت نوع 2 به بررسی­های بیشتر نیاز دارد. بنابراین ما یک مرور سیستماتیک و فراتحلیل را برای بررسی اثر HIIT بر روی نشانگرهای التهابی در بیماران دیابت نوع 2 انجام دادیم. روش شناسی: PubMed، Scopus و Web of Science تا 23 اردیبهشت 1401 برای مطالعات HIIT در مقابل کنترل یا HIIT تنها بر روی نشانگرهای التهابی مانند CRP، IL-6 و TNF-a در بیماران مبتلا به دیابت نوع 2 جستجو شدند. جستجو با استفاده از کلمات کلیدی HIIT، التهاب و دیابت نوع 2 انجام شد. اندازه اثر تفاوت میانگین استاندارد شده (SMD) و فاصله اطمینان 95٪ با استفاده از نرم افزار CMA2 محاسبه شد. یافته‌ها: از بین 257 مطالعه مستخرج شده، یازده مطالعه شامل ۲۵۷ شرکت‌کننده (143 نفر زن و 114نفر مرد) با محدوده سنی 44 تا 71 سال وارد متاآنالیز شدند. HIIT اثر معنی داری بر کاهش TNF-α [01/0p= ،(13/0- الی 12/1CI: -) 62/0-]  و CRP [02/0p= ،(08/0- الی 21/1CI: -) 65/0-] در بیماران دیابتی نوع دو دارد. با این حال، شواهد قابل توجهی برای اثر HIIT بر IL-6 [10/0p= ،(06/0 الی 70/0CI: -) 31/0-] بیماران دیابتی نوع دو وجود نداشت. نتیجه‌گیری: این نتایج نشان می دهد که HIIT می تواند باعث بهبود سیتوکین های التهابی و همچنین CRP در بیماران مبتلا به دیابت نوع 2 شود.

چکیده [English]

Aim:  Type 2 diabetes is associated with increased systemic inflammation that may be increased cardiovascular diseases for which exercise training is a potential therapy. The effects of high intensity interval training (HIIT) on inflammatory markers in Type 2 diabetes patients, however, require further elucidation. We therefore performed a systematic review and meta-analysis to investigate the effect of HIIT on inflammatory markers in Type 2 diabetes patients. Methods:  PubMed, Scopus, and Web of Science were searched up to May 2022 for HIIT vs. Control or only HIIT studies on inflammatory makers i.e. CRP, IL-6 and TNF-α in Type 2 diabetes patients. The search was performed using the HIIT, inflammation and type 2 diabetes keywords. Standardized mean difference (SMD) effect sizes and 95% confidence intervals were calculated using CMA2 software. Results: Among the 257 extracted studies, 11 studies including 257 participants (143 women and 114 men) with an age range of 44 to 71 years were included in the meta-analysis. HIIT was beneficial for promoting a reduction in TNF-a [-0.62 (95% CI: -1.85 to -0.13), p = 0.01] and CRP [-0.65 (95% CI: -1.21 to -0.08), p = 0.02]. However, there were no significant evidence for an effect of HIIT on IL-6 [-0.31 (95% CI: -0.70 to -0.06), p = 0.10] in Type 2 diabetes patients. Conclusions:  These results suggest that HIIT could induce improvements inflammatory cytokines as well as CRP in Type 2 diabetes patients.
